It is helpful to have as much information as possible at the time of the initial appointment. To help with this, please bring the following with you to your initial appointment:

1)  Contact Details

You will have already completed this information when you contacted our service

2) Parent Information

• Intake Form, completed by one or both parents
• Consent Form

3) Teacher Information

Please ask your child's teacher (or somebody who knows your child well) to complete this Teacher Questionnaire.
