The focus of clinicians consulting from CDN is on face-to-face clinical consultations in a timely and structured manner.
Most forms or paperwork requests can be completed by your GP or other health care professionals. If forms and paperwork are required urgently we would suggest that you contact your regular GP practice who will have been provided a summary of the your most recent appointment to assist with completion of paperwork.
We recognise that for the children seen at CDN there is often a need to interface with other agencies. Additional forms and paperwork may be required to support this (e.g. NDIS and the Education System.) Clinicians are happy to assist with completion of forms and paperwork within your child's scheduled appointment. This ensures that the forms accurately reflects your child's current situation.
A clinician's relationship is with families and their children, not schools or other third parties. Forms or paperwork will not be processed at the request of schools or others. Contact from schools and other third parties will be referred back to the family to contact the CDN directly to meet confidentiality and privacy obligations.
When bringing forms to an appointment it is helpful to have as much of the information completed ahead of time as possible, such as your child's name, date of birth and other personal details. If you are wanting to provide forms ahead of your appointment please ensure they are sent through 14 days in advance so they are ready for your clinician to review during the scheduled appointment time.
