Children in Departmental care

For children who are in the care of the Department of Community Services - Child Safety, we need additional information and authorisation before we can proceed.

1. The first page of each questionnaire is an authorisation that we need before we are able to finalise the appointment.
2. The subsequent pages allow us to collect background information about the child.
