** Parents - Please make sure you have the preschool / school questionnaire filled in by your child's teacher before your initial consultation with us, so that you can bring it along (or the teacher can send it direct). If we do not have this it makes our work less effective for your child.

Questionnaire if the child is in the care of the Department of Families

** Parents / caregiver. If the child is in the care of the Department of Families, please make sure all this questionnaire has been completed by the relevant departmental officer or manager, so that it is available to bring along to the first consultation.
