So far we have

1. A summary of your child (to help people understand)
2. A list of people involved (your team)
3. A vision of what you are trying to achieve (your goals)
4. A summary of your work with the CDN

Once you are underway, the team is involved, and you are working towards your goals, you need a way to track progress. These forms are to help you.

On these forms you can write progress notes, which may include:

• Important meetings
• Important decisions
• Progress achievements
• Strategies that work (and do not work)

Again, the PDF is for handwritten notes, the DOC is for wordprocessing. The format is suggested only.
